Unless there's some disciplinary action pending you just fill in a form and (usually) send the licensing agency a fee and in some cases proof that you attended continuing ed classes. That's just an administrative function having nothing to do with whether you should be practicing in your regulated field; you stay on the roster as long as you've registered, attended the classes and sent in the fee. The state can't deny a renewal just because the person is a loon unless they also have been disciplined or struck off for professional misconduct.
