Does he dress like that because of his medical issues or is it by choice? If he has difficulty putting on pants and shoes and needs loose clothing to dress himself I can understand.
Clothing isn't everything, but it does show professionalism and standards. Right now my workplace is in the middle of an election between two unions. The representative for one union dresses in hoodies and what looks like pajamas when he appears in court and important meetings representing union members. If you were fighting to keep your job, and your representative came into court wearing a pajamas that would be bad.
Congress is no different. Lives, jobs, future of the nation depend on what they do.
