One of the experts quoted in the op noted something very important, which is the extent and rate at which he's deteriorated. It's really very noticeable, once you become aware of the signs, but people miss it, because he's always been so weird.
For example, he's always been a huge liar, so when he says something that is obviously nonfactual, it's easy to shrug it off as "Oh, there he goes lying again, what else is new."
That's just one very small example, only one part of a much larger picture. If you want to know more, I recommend googling Dr. John Gartner, founder of A Duty to Warn. He's done a ton of interviews about this, and it's very easy to find videos of him expounding on it in detail. Here's a very recent one, where he talks about Trump's pre-existing personality disorders and gives examples of how Trump is exhibiting serious signs of dementia and having more and more trouble communicating coherently:
