It's one thing to wear them due to a health or medical issue and it's normal. I'm sure you have it under control as an independent adult.
In Trump's case, it's alleged his incontinence is egregious due to his poor diet, and that it was somewhat self-inflicted due to his past drug abuse. There are stories from production crew that they would have to temporarily shut down tapings of The Apprentice because he would soil himself, and there were designated people whose job it was to clean him up and change him like a baby while he stood there. And because he's used to this treatment, he will soil himself and sit there in it since he doesn't want anything to appear amiss, and people near him have to suffer because of that until he can get one of his handlers to do it for him.
I don't think it's a slam on him needing to wear incontinence garments, it's a slam that he's a rich spoiled old man who requires people change him like a baby and that it has to be a secret covert thing because he would never be honest about his own health issues and is constantly trying to project this "healthy man in his prime" image that is patently not true.
