12. Happens all the time.

My wife and I were driving down the road leading to Terminal 1 at MSP airport. We were following a box truck. Suddenly the top of the truck struck a large swinging sign announcing low clearance ahead. The truck continued on without noticing that it had hit the sign. The sign fell on the roadway, not far from the front of our car. Fortunately, I had been keeping back from the truck and was obeying the 15mph speed limit, so I stopped before hitting the sign. After we continued, we spotted the truck, which had sheared its roof off at the point the sign was warning about. Many flashing lights were on the scene.

Failure to pay attention.
