Insurance tends to be linked to geography, unless you work for a multi-state corporation.

COVID vaccinations were free for all because of government funding, although they billed your insurance of you had any. Presumably the government funding is gone for those not in the recommended list - so cost will depend on coverage by insurance company. If you can't get medical care in another state, you likely won't be able to get vaccinations in another state.
