apart from that.. wow on the kitty. I would train using
the pinch of the neck like a momma cat, remove physically/
change the focus of the kitty.. it may only be a partial
solution but often is very effective. (I have taught a
cat not to bite and scratch me that way.)
Also, tons and tons of physical stimulation of the
whole body, firm massaging and holding, akin to
the grooming and firm licks of a mother -- important
for brain development in early months.
There is a book called "Think and Grow Rich" by
Napoleon Hill, in which he tells an amazing story
about his son, who was born without any external
ears. At some point he observed his son responding
rhythmically to music while holding on to the
music machine, whatever it was.. and he realized
that he could 'hear' through feeling. After that
he began talking to his son by holding his chin
bone against his son's skull when he spoke, so
that the vibrations of sound were felt directly
in his son's body/head.. long story short, this
son learned to hear.
As I heard the story, what he talked to his
son about was that he could do anything;
that nothing was impossible. This guy was
very much into the power of positive thinking,
one of the pioneers probably.
