38. What accounts for reports of higher premiums thus far?

I understand that many of the important provisions of the law don't kick in until 2014, yet I always hear people complaining about how their premiums have increased, doubled, etc., in the last couple of years. Is this the private market milking people before those provisions kick in? I don't want to assume that, but that's just my theory. Does anyone have more concrete info? (I don't recall the specific situations or cost increases of the people who were doing the complaining.)
