I don't have to worry about any of that. I can't get insurance without the ACA, so a 'doctor/patient relationship' isn't anything I've had to concern myself with. The doctors in the ER don't seem terribly interested in building that relationship, either.

I have several customers that are doctors as well as two clinics. The two doctors I had time to discuss the ACA with were both very excited about it. One actually went off on another of my customers because the latter claimed it was going to destroy our 'great' healthcare system. Dr. Customer told him that his claim couldn't be further from the truth. That he was sick and tired of insurance companies telling him what he could do for his patients. That the ACA would ensure that his patients could be covered for preexistings and not have their coverage cut off after a 'lifetime cap'. He also mentioned that due to insurance, most of his patients wouldn't come in for well visits and would wait until they could no longer stand the pain and suffering they were going through. He moaned that it was often too late and their insurance wouldn't cover the catastrophic costs of late treatment.

That's the kind of doctor that is truly concerned with the 'doctor/patient' relationship. He didn't drive a Caddy, we laughed once about his lack of a yacht (inside joke about another local doc that had written a scathing LTE about Pres. Obama and the ACA). Oh, and his assistants adore him.
