In some cases it is covered by state law/regulation, in some cases not. It is entirely possible with some policies in some states to discover that if you have to travel to another state, you are either uncovered or up for a 50% copay because you are by definition out of network.

Medicare for All is the solution, and Democratic candidates should be campaigning on that.

Unfortunately even with Medicare doctors are not required to accept Medicare
