The other way of looking at it is they believe THEIR government program is going to be cut or defunded in order to pay for SOMEONE else's government program.

The interpretation - that they do not know Medicare is a government program - is merely an assumption being ascribed to the holder of the sign. The holder of the sign may very well know that Medicare is a government program, but has been led to believe it will be cut in order to pay for a broader program of "socialized medicine".

The interpretation ascribed to the sign is not the only one, nor is it a necessary implication.

But old people are ao dumb and confused that it is easy to make that interpretational assumption because, well, we all know how mixed up old people, and women in particular, are.
