1. I've known people that this has happened to...

and it's just a nightmare to sort out. I'm actually having a very tiny issue with it at the moment where for some reason, no matter how my doctor submits my visits to the insurance company, they reject it twice, then pay for it. Luckily, his office is used to dealing with them and it's a tiny amount, so it's not a big deal. However, what if they were less understanding and the bills were more?

Years ago, I had surgery and the anesthesiologist didn't get paid. Now THAT was a bill. Her people had submitted the bill "wrong" to the insurance. It eventually got worked out, but ... it was really scary there for a while because that was a HUGE bill, especially for me at the time.

So, my point is, yeah, this is a problem. It's overly complicated and just in overhead, it's wasteful.
