During the entire Cold War, the Soviets didn't nuke us because it would result in their country being wiped out however much damage they did to us.

At best, they could hope to die knowing they did about as much damage to us as we did to them.

For any other country, to nuke us or give terrorists nukes that are detonated here makes about as much sense as trying to go on a shooting rampage in a police station. You make take out a cop or two, but you will certainly end up dead.

In the case of a country nuking us, they might have a few minutes or even seconds to gloat about taking out one or a couple of our cities before their whole country no longer existed.

No matter how evil the leader of any other country might be, none would take an action that would have 100% chance of resulting in national suicide.
