There are multiple online places that will let you upload your own design, but some of them are pricey.

Or, you can do this: Print your design on a large shipping label (Avery has several sizes). Then cover the label with transparent Contact paper. Best to apply the sticker to a glass surface. The ones I've done have lasted a year or longer.
