4. Insurance companies can already operate across state lines...

It's just that they have to deal with the State insurance commissioners. What they want (and what that really means) is to dismantle that level of regulation on their activities so they can initiate a race to the bottom for rates and services. One thing about the ACA is that it provides national standards so that the states can't allow companies to get into bidding wars for the insurance companies by shredding their regs.
