9. I've already told my family not to put me in charge if they expect to be allowed to die.

I will not uphold any family members wishes to "pull the plug". If I am in charge I will override their wishes and order treatment to continue.

You might vigorously disagree with how I would act but I think that the disclosure in advance is fair. They can designate someone else to be in charge.

That said, I respect every other family and persons wishes to have done what they want. Just don't put me in charge of it.
