in 2004 my wife and I voted on a DRE that flipped a vote. We called the election supervisor, eventually got escorted out, and the machine was "reset" and put back in business.

It still happens in Florida - if you look at undervotes, patterns, etc. it's pretty clear that some DRE's or tabulators are hacked.
