What if a local medical provider got irritated about MSF providing free care (thus taking away customers) and somehow managed to get the Afghan government to claim that terrorists were hiding out in the hospital?
Boom- competition eliminated!
((It makes as much, or little, sense as other "why this happened" theories...))
