4. Part of it is the way that ad auctioning works

If you have a popular site that gets a lot of clicks, you can sell advertising space on it.

So a fake news page that is sensational and gets a lot of clicks earns a lot of money from advertisers. The kids in Macedonia are interested in money, not in some ideological program to interfere in US elections.

Since the US is an advertising oriented country, we are going to be bombarded with more fake news than one that is not a rich target for advertisers. Macedonia should be pretty immune to fake news.
