All federal civil servants/employees can, and most do, get their healthcare through the Federal Employees Health Benefits (FEHB) program. It's a marketplace with plans offering benefits based on a call letter sent yearly by OPM (office of personnel mgt). There are state-specific plans and nationwide plans ranging from HSA type plans to HMOs to FFS to BC/BS. The federal gov't generally picks up roughly 75% of the plan's cost with employees paying the balance. There's a yearly open season in Nov-Dec when you can switch plans. These aren't exactly gold-plated policies, but they run the gamut from ok to very good. There's also seperate dental, vision and long-term care insurance plans offered.

IIRC, when Obama was first elected to the senate he didn't enroll because he was on Michelle's insurance that was better.
