Speaker and President Pro Tempore aren't automatically promoted. If it were like that the president pro tempore of the senate would become speaker of the house.

Ryan only becomes president if both Trump and Pence are gone at the same time. Otherwise they select a replacement and that replacement is promoted.

That's how Ford became president, he wasn't speaker of the house. Nixon selected him after Agnew resigned. Rockefeller wasn't speaker either, Ford picked him.
