5. Contraceptives have almost always been covered when prescribed to treat a medical condition.

Covered to treat such conditions as dysmenorrhea and amenorrhea and the like because the purpose of medical insurance is to treat disease and injury. Since the desire to avoid pregnancy is neither, most policies didn't cover them under that circumstance. Then again, over the last 10 or 15 years even before ACA more and more states mandated coverage for contraceptive purposes so they were covered.

If ED meds are covered, it would be because they are a treatment for a legitimate medical malfunction.
