I'm not saying the insurance is great but they are the National Association for the Self Employed and offer private policies that are a little more affordable. We've had a family policy with them for years. Our agent said that with a pre-existing condition he signs up the family and then after 6 months adds the person (with the pre-existing condition) to the policy. I haven't tested his words but it might be worth a try.

I would argue that it isn't taxable income since it was the policy holder that paid it originally.
