I doubt he sees any downside at all.

In all likelihood, he probably figures that an all out first strike would eliminate their ability to retaliate with what few weapons they have.

Whether they pound the crap out of Seoul or Tokyo is not his problem.

He is a sociopath. Consequences to others do not factor in to his decision process.
