13. Where were the lookouts? There are supposed to be people reporting what their eyes are seeing.

See a target, see red or green, white mast lights? How many points off the bow, beam, stern, port or starboard?
This is just basic shit. Maybe they're reporting this information but the officer on duty isn't processing it.
Whatever it is, something is very, very wrong.
