Romney and Ryan woujld get rid of Obamacare, go back to the death panels by for profit insurance carriers, denying pre-existing conditions and put a cap on benefits even if people die. Yet Lyin' Ryan is busily diverting seniors in FL by using the Sarah Palin federal death panel scare.
Right now, GOP legtislators deny women equal access to insurance because they are asking for birth control, etc. in states where they have the majority. Those are aspects of the ACA that Romney can stop on Day One, and that Ryan has defunded in Congress already and Obama had to work around.
GOP governors have blocked Medicaid payments to Planned Parenthood because it is a joint state and federal run program. To keep essential healthcare for women who couldn't afford it elsehwere, Obama provided the money through Title IX:

No person in the United States shall, on the basis of sex, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any education program or activity receiving Federal financial assistance...

Because of Republican Congressional obstruction and radical GOP governors, Obama had to take that step despite the passage of the ACA. AFAIK, he shifted available funding to wellness care for women under Title IX.
So whoever is POTUS can make this happen or not, because of the flexiblity built into the ACA allowing states to refuse to participate. Although someone else may remember the law better, there are provisions for exchanges, but the same GOP governors have refused to set them up.
It is imperative that Romney not get into office. I believe Ryan is correct there.
