3. Here in the Philly area, we had 2 suburban counties who already started offering it

right after the FDA approval. The other surrounding counties (and the city) said they would wait for the CDC approval (probably out of an abundance of caution for anyone who might have questioned giving it without the CDC review and announcement).

As a note in your situation, I think most "non-child-targeted" websites require users be age 13 and older to access the site, so that might be why they require a parent (or guardian) to do it (and using the phone, they might end up sending you an email with a link to use to schedule the appointment or would do it over the phone).
