Simple!

I'm guessing some home health care workers are cashed up and don't need the job, or just aren't very bright.

My money is on the latter.

The word on the street here is that nurses who don't make the grade go in for working geriatric nursing homes, where the work is/can be dirty but the job spec requirements are lower.
Those who can't make it there, go in for home care, maybe working as temps for different agencies. Where standards required are questionable.
