They believe this case came from a live-attenuated vaccinated population. I don't quite understand the mechanics, but live-attenuated can cause polio in 1/1,000,000 cases and then it can possibly spread to unvaccinated populations.
(I assume the polio case from the vaccinated person is an asymptomatic case)
