Afraid, complacent, and eternally grateful for a few scraps of food and a roof over head.

Americans are already used to not have access to healthcare.

Also dictators usually don't believe in mental health diagnosis or treatment.
The only real mental illness in their book is when people don't believe they are
god and supreme leader.
