This is what would have taken place in every state had the Republican leaders had their states expand medicare and set up their states exchanges. They didn't do this because they knew that this would slow down the rollout and place the load on the main government exchange. We will see that in the future they will have no choice but to get on board and go with it for the people will see that this is good for them.
