The ACA required all employers with more than 50 employees to offer health insurance and pay at least half the premiums for employees working 30 or more hours. While it was often customary, it was never required before. The insurance offered also had to comply with the requirements of the ACA. I know, for example, when I worked for Whole Foods that while their health insurance was pretty good, it didn't cover mental health except for prescriptions.
