I searched, but didn't have enough information to go on. Yes, I can see now that the ladder is the outermost structure on the train. The conductor's yell indicates to me that he thought the kid was too close and that the kick wasn't just punitive. Though, I agree the kick wasn't placed perfectly and thus may have increased the danger. I just hope there is a lesson in the laughter.
