1. Failure to provide the requested documentation.
2. You have an outstanding warrant, are required to appear in court, are serving time for a felony, or have been convicted of international drug trafficking.
3. You owe more than $5k in back child support.
4. You are not a US Citizen.

As much as I hate to say it, Bush being President had nothing to do with it.
