Get the blue states to work together to create interstate single payer pacts? Under the ACA, they should be able to do something like that, I believe. Get it up and running in a couple of states and work to get blue state after blue state to join up.

And it doesn't have to be just healthcare. Start a 'Blue State bank' owned and run by the states involved. Get healthcare and banking into public hands.
