2. I was safety manager for a recycling company.

It doesn't matter how important the safety instructions and regulations are, there will be violations because of expedience or the stuff isn't comfortable to wear. On the other side of the equation, equipment fails and people do make mistakes.

The simplest safety rule is, "If it looks stupid, don't do it."
