1. Touch pad still active - touching it with thumb? Bare (no detail) spots on mouse pad?

You have the Check mark in "Automatically move pointer to the default button in a dialog box"? (Control Panel > Mouse > Pointer options)

Those are the 3 biggies.

Then there is hair and stuff on the bottom of the mouse where the LED is.
In a ball mouse, hair and stuff inside the mouse!
