First, eliminate viruses and prions. Make it impossible for bacteria to negatively impact the health of multi-cellular organisms.
Since we are talking about a fantasy world, other issues, make it physically painful for one sapient to harm another. Strengthen DNA to make harmful(cancerous, etc.) changes in it impossible. Eliminate all harmful mutations in all genomes. In addition, harmful parasiticism will no longer be allowed.
Hmm, natural disasters will no longer happen.
Running out of ideas, but eliminating disease and suffering seem to be up there.
