It's not that I'm all Twitter literate but here goes.

There are two basic things, the @ and the #

The @: This is what you use when you are replying or sending a tweet to someone specific, it appears on their feed. @BernieSanders I don't think that capitalization matters.

The #: This is what you use to denote a subject. It's kind of like identifying a keyword. These are what drives the trending list. #FeelTheBern is one popular one for posts about Bernie. Hashtags are arbitrary and include everything from the hash itself through the next space. If you search a hashtag you'll get a list of all the tweets that contain it.
