1. If the investigation is ongoing then how does one justify the use of "accidentally"?

Wouldn't the investigation determine whether that term was appropriate or not?

One could say "This happened; we are investigating how/why." Adding descriptors which one isn't prepared or able to justify at the moment they are used isn't good from a credibility standpoint.
