I think his foot ends where that weird creased looking spot is. The toes might or might not go beyond the crease. His lifts are like putting on a high heel in a flat. And because the front of the shoe doesn't have a foot to fill it, it's perfectly unblemished, while the rest of the shoe is distorted from the tiny foot and the lift.
