"It doesn't show subtle cognitive decline" is being taken to mean "it overlooks moderate to moderately-severe dementia".

There are ways of testing for various kinds of dementia years before the first symptoms become obvious, but they're complicated, not always accurate, and, again, test for dementia years before the first symptoms become obvious. In other words, they help explain some small, almost trivial changes. Not the huge deviances from acceptability with Trump.
