The exam the doctor did obviously was nothing more than the superficial going-over anybody would get during a routine doctor visit and it would not necessarily detect evidence of underlying heart disease or other afflictions that don't always manifest themselves in early stages. Trump is infamous for hating to be touched (unless he's porking a porn star) so it's likely the doc's rubber-gloved fingers didn't find their way to certain areas at all. Betcha Kroner to Krugerrands he didn't strip down past his underwear (or even that), and recited his own fake height and weight statistics - and the doctor, who is a naval officer under the command of the Commander in Chief, just wrote them down and didn't argue.

It's not uncommon for people (especially old, fat, sedentary ones) to leave their doctors' offices after having received a clean bill of health, and drop over dead from a coronary the next day.
