who have had IVs. Looks to me like both hands. There's a bandaid on his left hand, too, and a wound closure strip on his right.
They had to switch hands for the IV, it looks like to me. Remdesivir, perhaps, which is administered through an IV. So are the monoclonal antibodies Trump received.
That's my guess.

Yes - At his age, he's most likely on blood thinners. Blood draws & IVs will cause this effect.

Gotta agree. It certainly appears to be hospital procedures on both his hands/forearms.
