They always maintain that only M.D.s should be given the title “Doctor.”

As an example, when my wife and I got married, we had to research the wording of the invitations, and found that my father-in-law, who held a Ph.D. in physics and worked as a consultant in the energy industry, had to be listed as “Mr.” rather than “Dr.”.

Now, when it comes to the world of academia, different rules apply, and it would be standard practice to refer to the future FLOTUS as “Dr. Biden” in the context of her work. But, outside of the academic world, traditional practice would say she shouldn’t be referred to by the title.
