Health care shouldn't get into such questions.

I understand the frustration with these people, believe me. I just believe health care is a fundamental right and immoral or unethical behavior should have nothing to do with it.

Private insurance, if they have it, they presumably paid for it or earned it through their employer. Public insurance, they paid for through their taxes. But even if it's unpaid, it shouldn't matter. Someone who lives off completely off the grid and never pays even sales tax should still get health care.
