I suspect a combination of biological and environmental factors. We know some of the disorders I listed have a genetic component and run in families. The addictive disorders and bi-polar. There is some research about the genetic factor in personality disorders but not conclusive.
Mary Trump talks about the cruelty of Trump's father. So we have a cruel, abusive father, and absent mother during a critical phase of early psychosocial development. Then the genetic factors in play as well.
I think that was the pattern growing up. Absent mother, cruel and abusive father. Biology also part of the picture.
So a child with no sense of right or wrong, no empathy. Entitled, self centered. Prone to violence and rule breaking.
And parents who allowed it or even encouraged it. Their wealth covered it all up. None of this did Trump any favors.
With therapy even as a teen he might have had a shot at some normal life. Today there are residential treatment centers or day treatment for the conduct disorders (child sociopath) that can make some progress.
But sociopaths are notoriously hard to treat and outcome is not good.
