3. Good lord, it's very rarely the pharmacist that's a problem with getting a prescription...

It's typically either the insurance or middlemen pharmacy providers requiring endless iterations of pre-authorization for.expensive or regulated drugs, deciding they know better than your doctor or changing up what's on their formulary(Express Scripts is horrible for that...) or the pharmaceutical companies themselves with their increasing amount of shortages or seasonal production of generics in the name of profits.
Not to mention the pharmacies not employing enough pharmacists or techs to cover known surge times or their "add on" health care services (vaccines, simple wellness care).
The only time the pharmacists themselves have been at fault with was not checking the vault when a controlled or psychotropic medications come in and the patient is desperate due to having run out.
