I am a retired mental heath provided who practiced in a state with Duty To Warn and Protect.

For depressed patients with no suicide ideation or history, a talk about the increased lethality for suicide with guns in the home and a recommendation that guns be removed as a precaution.

For an adult with depression, suicidal thoughts or history, a direct request that guns be removed from the home.
A call made to next of kin with my concerns and documented.

For a child with depression, angry, thoughts of suicide, homicide and guns in the home, parents are called immediately to come to the office. It is made clear guns must be removed from home immediately.
Recommendation that child be hospitalized inpatient psych.

If there are direct threats to specific potential victims they are notified.
If there is no proof of guns being removed a call to children services and or police.
