A military attache is with Trump at all times, carrying a briefcase referred to as the "football". Trump carries a card with authentication codes on them referred to as the "biscuit". If he wants to order a nuclear strike he summons the attache, provides the biscuit, and is then given the codes from the football. After that he can then order the strike by providing the codes from the football.

He doesn't need to ask the Chairman of the Joint Chiefs for the codes. He has ready access to them at all times without the Chairman being involved at all.

This story is not plausible, and it's not because Trump isn't stupid and/or crazy enough to order an unprovoked nuclear strike. It's because our nuclear command and control procedures don't work the way the story describes.
